As a result of the discussion at the November 2024 Parkland Libraries Council (PLC) meeting, it was decided that the Damaged Item Procedure and corresponding webform on the PRLS Support Site needed a bit of a reboot in order to incorporate lost items. These updates have now been made, with a handy Initial Steps section added to the procedure (available on the Support Site here or in the Q Drive here: Q:\Polaris\Damaged or Lost Item Procedure.pdf). The Damaged or Lost Item webform, found under the Forms drop-down, has also had some tweaks.
Please remember that when you fill out this webform Parkland staff will forward it to the appropriate libraries, but it is the responsibility of the libraries involved to resolve the issue together. You can do this by filling out the form, emailing or calling the owning library directly.
